Step Two — Application for Enrolment

A Quiet Beginning, Gracefully Made

Now that your lesson day and time have been arranged with Ms Jin Loh personally, the Enrolment Form below allows her to gently come to know you a little better as her new student. The more she knows you — what you enjoy, what your work or studies involve, what familiar references resonate with you — the more thoughtfully she can shape each explanation, each analogy, each musical phrase, to suit you in the lessons that follow.

A Gentle Note on Timing Kindly complete this Enrolment Form in full at least three (3) days before your first lesson day, and kindly settle the full first month’s lesson fee at least one to two (1–2) days before your lesson commences. Both serve as gentle confirmation of the lesson slot Ms Jin Loh has reserved for you.

Once you have read and agreed to the Studio Rules and completed this Enrolment Form, the next step — the Online Lesson Setup Guide — will gently open, designed to help your online lessons with Ms Jin Loh unfold smoothly, comfortably, and as enjoyably as possible.

How would you most kindly wish Ms Jin Loh to address you in conversation? A first name, a title and surname, or a name of your own choosing — whatever feels most natural to you.
As is standard practice in established music institutions worldwide — and as one would present at a doctor’s clinic or any professional consultation — a National ID or Passport number is required for the studio’s records. This information is held in confidence and used solely for the studio’s administrative records.

In the rare event of an unforeseen situation during a lesson — for example, a sudden faintness, an asthma episode, or any urgent matter requiring immediate attention — Ms Jin Loh would wish to know who to gently contact on the student’s behalf.
